ABOUT THE ROLE
As a Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure that optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your role is essential in improving patients' quality of life by delivering care over the phone and through virtual platforms.
WHAT YOU’LL DO
In this role, you will provide direct patient care through virtual platforms and deliver high-quality care to meet each patient's unique needs. You’ll provide comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care, offering clinical and care coordination as needed. This is a full time, remote role. The hours are Monday through Friday from 8:30 AM to 5 PM Eastern Time.
Compensation for this role is anticipated to be in the range of $85,000 to $100,000 annually, depending on location and years of experience.
