As a Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure that optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your role is essential in improving patients' quality of life by delivering care primarily over the phone or and through virtual platforms. You’ll also have the opportunity to help patients enroll in Daymark, accessing our services and support.
WHAT YOU’LL DO
In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs.

Compensation Range:
$85,000 - $100,000 USD
